Community Health Survey
Help us improve! We are conducting a community-wide health survey for the 6 counties in our region. We would love your feedback on the health and wellness needs of our communities. The survey will close September 23, 2019.

We envision a public health partnership that protects the people and environment in our six county region, promotes health and prevents disease, resulting in optimal health for all our citizens as our region grows and becomes more diverse.
